Just another failure of intelligence? Or something more sinister in intent?
Anyway the USA are the last people to point the finger about cyber-warfare since they (or perhaps their proxy, Israel) have been responsible for planting viruses aimed at Iran.
Plus all the ado of the NSA spying on both friendly and enemy powers for both national security and commercial purposes.

North Korea does not have the capability to pull off something like this (this is my area of professional expertise). At best, they outsourced it but I think most likely it was an insider.
Probably, it was another Edward Snowden. North Korea does not even have the type of rockets that the US government is claiming that they have
